By completing the registration form, you acknowledge you have read and agree to the terms of the following documents:

Waiver and Release of Liability
Please read carefully. This is a legal document.
In consideration of being allowed to participate in any way in the Tucker MS Tigers Baseball athletic sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child’s participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove my child from participation and bring such to the attention of the nearest official immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Tucker MS Tigers Baseball, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
By completing the registration form, I acknowledge that I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by agreeing to it, and agree freely and voluntarily without any inducement.
Consent for Medical Treatment
This form provides authorization for emergency medical care.
I, the parent or legal guardian of the player being registered, do hereby grant my consent for the Tucker MS Tigers Baseball, its coaches, assistant coaches, league officials, and other volunteers, to seek and obtain emergency medical attention for my child in the event of any injury or illness that may occur during any Tucker MS Tigers Baseball-sanctioned activity, including practices, games, and team events.
This authorization is granted only in the event that I, or the listed emergency contact, cannot be reached in a timely manner. I understand that league volunteers will make every reasonable effort to contact me before any treatment is administered.
I authorize any licensed physician, surgeon, dentist, or medical facility to provide any necessary medical treatment, including but not limited to, first aid, x-rays, anesthesia, and surgical procedures.
I also accept full financial responsibility for any and all medical expenses incurred as a result of such treatment.
By completing the registration form, I acknowledge that I have read and fully understand the terms of this medical consent form and voluntarily agree to its provisions.
Media Release Form
This form grants permission for the use of player images and videos.
I, the parent or legal guardian of the player being registered, hereby grant the Tucker MS Tigers Baseball the irrevocable and unrestricted right to use and publish photographs and/or video recordings of my child for editorial, promotional, and advertising purposes.
This includes, but is not limited to, use on the official Tucker MS Tigers Baseball website, social media channels (e.g., Facebook, Instagram), brochures, flyers, and other promotional materials. I understand that these materials may be used to promote the league and celebrate its players and activities.
I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, are and shall remain the property of Tucker MS Tigers Baseball.
I hereby waive any right to inspect or approve the finished product, including written copy that may be created in connection therewith. I understand that no compensation will be provided for the use of these images or videos.
By completing the registration form, I acknowledge that I have read this release and agree to its terms.

Concussion Protocol & Safety
Player safety is the highest priority for the Tucker MS Tigers Baseball program. We adhere to state laws and CDC guidelines regarding head injuries.
Heads Up: When in doubt, sit them out.
It is better to miss one game than the whole season. We empower our coaches to always prioritize safety over competition.
Our 4-Step Action Plan
1. Recognize the Symptoms
Coaches, parents, and officials are trained to look for signs of a concussion after any impact to the head, face, neck, or body. Symptoms may include:
- Headache or "pressure" in the head
- Dazed or stunned appearance
- Nausea or vomiting
- Balance problems or dizziness
- Confusion about assignment or position
- Answers questions slowly
2. Remove from Play Immediately
Any athlete suspected of having a concussion will be immediately removed from all league activities (practice, games, warm-ups). The player will not be allowed to return to play on the same day.
3. Refer for Medical Evaluation
The player's parents/guardians will be notified immediately. It is strongly recommended that any player suspected of having a concussion be evaluated by a licensed healthcare professional.
4. Return to Play Protocol
An athlete who has been removed from play due to a suspected concussion may not return to any league activity until they have provided a signed clearance form from a licensed healthcare professional trained in the evaluation and management of concussions.

The Tucker MS Tigers Baseball program is committed to creating a safe and positive environment for all its participants' physical, emotional, and social development. We have a zero-tolerance policy for abuse and misconduct.
Our Commitment
In compliance with the "Protecting Young Victims from Sexual Abuse and Safe Sport Authorization Act of 2017," our program has implemented the following policies:
- Mandatory Training: All adults who have regular contact with or authority over minor athletes—including board members, coaches, and key volunteers—must complete abuse prevention training from the U.S. Center for SafeSport.
- Background Checks: All adult volunteers are required to pass a comprehensive background check as a condition of their participation.
- Confidential Reporting: We have established a clear and confidential system for reporting any suspected misconduct. Reports can be made to the program's head coach without fear of retaliation.
How to Report a Concern
If you have a reasonable suspicion of sexual or physical abuse, you must first report it to the appropriate law enforcement authorities. You can then report concerns to:
- The U.S. Center for SafeSport using their online reporting form or by calling (720) 531-0340.
- The Program Head Coach via the confidential email: tmstigers.baseball@gmail.com.

Being a Tucker Tiger is about more than just playing baseball. It's about being a good teammate and respecting the game. All players are expected to follow this code of conduct.
As a Tucker Tigers Player, I will:
- Always play my best and have fun.
- Be a good sport, whether we win or lose. I will cheer for my teammates and congratulate the other team.
- Show respect to my coaches and listen when they are talking.
- Show respect to the umpires and accept their calls without arguing.
- Never bully, taunt, or make fun of any other player—on my team or the other team.
- Control my temper. I will not throw my helmet, bat, or glove in anger.
- Be on time for practices and games.
- Work hard to learn the rules and improve my skills.
Consequences
Players who do not follow this code may have reduced playing time, be asked to sit out a game, or in serious cases, be removed from the program.
Parent Code of Conduct
By registering a child with the Tucker MS Baseball program, parents and guardians agree to abide by this Code of Conduct. Our goal is to create a positive, supportive, and safe atmosphere for all participants. Remember, the game is for the kids.
I hereby pledge to provide positive support, care, and encouragement for my child by following this code:
- I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and officials at every game, practice, or other youth sports event.
- I will place the emotional and physical well-being of my child ahead of a personal desire to win.
- I will remember that the game is for the children and not for the adults.
- I will do my best to make youth sports fun for my child.
- I will ask my child to treat other players, coaches, fans, and officials with respect regardless of race, sex, creed, or ability.
- I will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing, taunting or using profane language or gestures.
- I will let the coaches coach. I will refrain from yelling instructions or criticism from the sidelines.
- I will respect the officials and their authority during games and will never question, discuss, or confront coaches at the game field.
Enforcement
Any parent or guardian who violates this Code of Conduct is subject to disciplinary action, which may include being asked to leave the field, suspension from attending games, or, in severe cases, removal from the program without refund.

Player health is our top priority. The following pitch count and required rest policy is strictly enforced for all players.
